When the student has completed the intake process and becomes eligible for services, Student Access and Accommodation Services will provide the Student Access and Accommodation Services digital ID Card. The digital ID card will be sent to the student's university email address. The student then forwards the digital ID card to their faculty, via email. The digital ID Card verifies that the student has a documented disability and/or medical/mental health condition and lists the approved accommodations. This digital ID Card promotes an avenue for mutual discussion between the student and faculty about the impact of the student's disability and/or medical/mental health condition in the educational setting. The student is responsible for initiating contact with the faculty to make an appointment to discuss the digital ID Card and discuss the accommodations. This digital ID Card is effective until the student is no longer a student at Illinois State University or the accommodations change.
